About us
About Horizon Care Support
Horizon Care Support Ltd is developing a domiciliary care service for adults aged 18 and above, specialising in support for people with mental health needs, complex care needs and dementia.
Who we are
We aim to provide compassionate personal care and practical support that helps people live safely and comfortably in their own homes, maintain daily routines and remain connected to their communities.
Every care package will be individually assessed and accepted only where we have the skills, staffing and resources to meet the person's needs safely.
Our planned personal care service will begin only once the required Care Quality Commission (CQC) registration has been granted.
Our vision
To provide trusted, compassionate care that enables people to live with dignity, exercise choice and achieve the greatest possible independence in their own homes.
Choice, consent and human rights
We will seek consent before providing care and respect each person's right to make decisions.
Where there are concerns about someone's ability to make a particular decision, we will follow the Mental Capacity Act 2005. Any best-interests decision will consider the person's wishes, involve the appropriate people and use the least restrictive option.
Our care will respect cultural identity, faith, relationships and individual preferences. We will make reasonable adjustments and provide information in accessible formats.
Our staff and service reliability
We will recruit suitable staff through identity, employment history, reference, right-to-work and appropriate DBS checks.
Staff will receive induction, relevant training, supervision and competency assessments, including training appropriate to mental health, dementia and physical care needs.
Care schedules will allow sufficient time for agreed tasks and travel. We will promote continuity and maintain arrangements for staff absence, delayed visits and service disruption.
People receiving care will have clear contact details and information about support outside office hours during their agreed service provision.
Quality and accountability
Clear management responsibilities, secure records and regular quality checks will support our service.
- We will review care plans, medication records, incidents, safeguarding concerns, complaints and feedback to identify improvements.
- When things go wrong, we will be open, explain what happened, apologise and take appropriate action, including meeting applicable duty of candour requirements.
- Care reviews will consider each person's wellbeing, dignity, independence, healthcare access and experience of support.
